Dr. Michelle O’Brien is a family physician with a masters in public health in maternal and child health.
She grew up in Central Minnesota and has lived in the metro off and on for the last 30 years. She practices full spectrum primary care but has a passion for women’s health and pediatrics. She utilizes lifestyle medicine to help patients prevent and reverse chronic disease, helps patients decrease their exposures to toxins and supports strong mental health and resilience using positive psychology, mind body practices and Brené Brown’s Daring Way curriculum. In her free time she enjoys spending time with her family (including their golden doodle puppy), traveling, supporting social justice causes, trying new cuisines and restaurants, music and reading.
Dr. Leslie Surbeck is a board certified internal medicine physician who provides primary care for adults age 18 and above. After graduating from the University of Minnesota Medical School in 2004, she completed an internal medicine residency at Hennepin County Medical Center in Minneapolis. She then served as a chief resident before joining the faculty there as a hospitalist physician. After several years in hospital medicine, she transitioned to outpatient medicine and has been practicing as a community primary care physician for the past 8+ years. Her clinical interests include preventative medicine, anxiety and depression treatment, and womens health.
Heidi Andrews has been working at Sage Prairie since May 2020. She received her PharmD from University of Minnesota in 2015 and completed a two year leadership residency program through the University of Minnesota in 2017.She recently became a Board Certified Ambulatory Care Pharmacist. Heidi currently lives in Robbinsdale with her husband, Scott, and a 1.5 year old Boston Terrier named Axel.
Sara is a Psychiatric-Mental Health Nurse Practitioner. She obtained her Associate’s Degree in Nursing at Hibbing Community College before moving on to obtain her Bachelor of Science in Nursing and Master of Science in Nursing at The College of St. Scholastica. She has worked in the healthcare field for over 10 years in various settings. She provides psychiatric consultation and medication management services to those suffering from a wide range of mental health concerns including mood disorders, anxiety disorders, and psychotic disorders.
